Recognising your individual living circumstances is key to designing a support plan that truly fits. As part of our process, we’ll arrange a home visit to understand your environment and explore how we can assist you or your family member in maintaining a safe, comfortable space.

Prior to the visit, we’ll send you an Intake Form to collect essential details. During the assessment, we’ll take a close look at your home setup, daily routines, and personal goals. This insight allows us to create a tailored support plan that aligns with your specific needs and priorities.

 At Tomorrow and Beyond, we deliver all assessments and interventions in line with your funding arrangements, including NDIS, Medicare, private health, and other non‑NDIS options. Our Sunshine Coast Occupational Therapy team works alongside people of all ages, helping them stay safe, active, and engaged in their community. We offer:

• Mobility and equipment assessments that promote independence and confidence
• Home‑based falls risk assessments and practical prevention strategies

• In‑home dementia support and COPE‑informed interventions
• Cognitive screening using tools such as the MoCA
